Requests to provide the seasonal influenza vaccinations at locations other than the pharmacy premises (off-site)

The Community pharmacy advanced service specifications – seasonal influenza vaccination  for adults and children include a requirement that pharmacies seek consent from the commissioner if they wish to carry out vaccinations at a location off the pharmacy premises (e.g. patient’s home, care home, long-stay residential facility or community venue).

If your pharmacy intends to provide flu vaccinations at locations other than the pharmacy premises, please complete and submit the following MS Form.

Please note a separate form is required for each location, with the exception of housebound, where you are intending to deliver vaccination to housebound patients, please complete one form and tick the housebound cohort.

Confirmation of the decision (consent /decline) to vaccinate off-site will be emailed back to your NHS pharmacy generic email address as soon as possible. We will aim to do this within 48 hours of receipt.

Whilst it is not a specific requirement within the service specification to notify an individual’s GP of plans to undertake off-site vaccinations, Pharmacy providers are encouraged to work collaboratively and proactively with GPs and other local partners when planning off-site activity to maximise the offer and reduce duplication.
