The Area Prescribing Committee would be interested in any problems experienced where there are communication issues  with shared care or other medicines related issues, particularly where guidelines are not being complied with, to the following email address: barnsleyapcreport@nhs.net.

There is a template reporting form available to use on PharmOutcomes that can be sent directly to the above email by clicking on Barnsley Interface Issue Report after saving the completed form.
