Record and claim form for Urgent MDS supply for DBHT

The aim of the scheme is to provide MDS trays to patients on discharge from DBHT Hospitals. The principle being that on occasion some patients discharge is being delayed due to the hospital pharmacy’s capacity to be able to provide an MDS device for a specific patient. The LPC appreciates that the urgent nature of these supplies could put your pharmacy under some pressure and this is why the LPC has secured agreement that the initial supply will be for seven days only allowing greater flexibility for the remaining 21 days supply. Because DBHT are discharging their responsibility for the provision of an appropriate adjustment, in this case an MDS tray, to your pharmacy they are paying £20 per patient for this service. This is a one off payment, because once discharged the patient becomes your responsibility and it is incumbent on you to undertake any further assessments under DDA (now Equality Act) for on-going supply of MDS trays. The service does not put any demand on a pharmacy to make on-going supplies of MDS trays.

CPSY hopes that all Doncaster pharmacies will participate in this scheme and therefore anticipates that there will not be any excessive burden on any individual pharmacy. It is expected that there will be no more than a couple of dozen or so of these referrals per month so many pharmacies will not be called on every month to make an urgent supply. However, it is within your right to turn down making a supply, but please bear in mind that if you turn a request down the hospital pharmacy will direct the request to another pharmacy of the patients choice.

Claiming 

Claims should be made as soon as possible (or at least by the 5th of the month) after making the MDS supply using the attached form and sent to DBHT for the attention of
