Hypertension Case Finding Service

The Hypertension Case Finding Service (HCFS) is an advanced which service aims to:

  • Identify people aged 40 or over—or younger if deemed appropriate by pharmacy staff—who may have high blood pressure and refer them to their GP for diagnosis and management.

  • Conduct ad hoc clinic or ambulatory blood pressure measurements at the request of a GP, for individuals with or without a hypertension diagnosis.

  • Provide opportunities to promote healthy lifestyle behaviours to patients.

In public-facing communications, the service is described as the NHS Blood Pressure Check Service.

UPDATED SERVICE SPECIFICATION – SEPTEMBER 2026

Changes to the Hypertension Case-Finding Service (HCFS) came into effect on 3rd September 2026, including revised patient eligibility criteria. Pharmacies providing the service should review the updated service specification and ensure their teams understand the new requirements.
Community Pharmacy England (CPE) has updated its suite of promotional resources, including patient leaflets, prescription bag flyers, referral templates, team briefings, social media assets and a service promotion checklist. Click here
CPSY has developed two concise guides summarising the changes, one for community pharmacy teams and one for GP practice teams:
We have shared the GP practice guide with practices across South Yorkshire, asking them to review the changes, update relevant searches, referral templates and signposting processes, and brief reception and care navigation colleagues.
Pharmacy teams should review the community pharmacy guide and ensure all relevant colleagues are familiar with the changes. You may also wish to contact your local GP practices to discuss how eligible patients will be identified and signposted to your pharmacy.
