Requests to provide the Seasonal Vaccination Services – COVID-19 and Adult Flu at locations other than the pharmacy premises (off-site)

The 2026/27 Community Pharmacy Seasonal Vaccination Services specification, covering COVID-19 and adult flu vaccinations, requires pharmacy contractors to obtain prior consent from the NHS England regional vaccinations team before providing vaccinations at any location away from their registered pharmacy premises. This includes patients’ homes, care homes, long-stay residential facilities and community venues.

If your pharmacy intends to provide flu vaccinations at locations other than the pharmacy premises, please complete and submit the following MS Form.

Please note a separate form is required for each location, with the exception of housebound, where you are intending to deliver vaccination to housebound patients, please complete one form and tick the housebound cohort.

Confirmation of the decision (consent /decline) to vaccinate off-site will be emailed back to your NHS pharmacy generic email address as soon as possible within 48 hours of receipt. Please Note – Submission of this form does not automatically mean approval of the request(s) has been given and no activity should be delivered off site without email approval from the NHS England regional team.

Whilst it is not a specific requirement within the service specification to notify an individual’s GP of plans to undertake off-site vaccinations, Pharmacy providers are encouraged to work collaboratively and proactively with GPs and other local partners when planning off-site activity to maximise the offer and reduce duplication.
